​Immigration Physical Exams



What you need to know
When you apply for a green card (adjustment of status) in the United States, you usually need to have a medical examination. The exam must be done by a doctor who is authorized by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (also known as civil surgeons) to perform the medical exam required for most Green Card applicants. Dr Leite-Evans is a certified civil surgeon. 

What to Bring to the Exam

You Must Bring the following to your medical exam:
  • Form I-693, Report of Medical Examination and Vaccination Record

  • Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

  • Vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease)

  • Payment - Cash payment required ​​

Reference and source

Process

  • 1. You or your representative calls to make an Appointment for Physical Exam
  • 2. After your Physical Exam the doctor will refer you to Quest Diagnostics laboratory for the required lab tests 
  • 3. When the doctor receives your lab results she will contact you for a second appointment
  • 4. At the 2nd visit, you MUST BRING Form I-693 (form can be downloaded from the USCIS website)
  • 5. ALL PERSONAL QUESTIONS MUST ALREADY HAVE BEEN ENTERED BY YOU on the form prior to your visit and the Doctor will complete only the MEDICAL section
  • 6. You will receive the completed form which you then submit to USCIS. A copy will be maintained for a limited time by the doctor's office.
